Appointment Request

Please note: For spay/neuter services, proof of rabies vaccination or vaccination at the time of surgery is required for cats that are three months or older.

Please fill out the form below to request a spay/neuter or wellness appointment for your cat. Appointment requests are reviewed and approved during business hours (7:30am - 3:30pm) Monday through Friday. Please note that we are in Madison, WI.

FOR SPAY/ NEUTER APPOINTMENTS

Under Step 2, if you are unsure of the cat’s Sex, choose ‘Female’. Give your best estimate for the cat’s weight. We weigh every cat before surgery and will update this field with the accurate weight.

Under Step 4, give your best estimate of the cat’s Age. For Breed, most cats are ‘Domestic Short Hair’

Under Step 5, please select which services you would like. You can also add and delete services if you select a ‘package.’ We will confirm these things with you at drop-off and if you have questions, please feel free to email us at clinic@madisoncatproject.org.

If you are looking to schedule multiple cats on the same day, you must schedule an appointment for each cat.

FOR WELLNESS APPOINTMENTS

If you are scheduling for routine care with no other medical concerns, please select ‘Wellness Appointment’.

If you are scheduling for a wellness appointment that involves other medical concerns, please select ‘Minor Medical Exam Appointment’.

Current patients: When scheduling a follow up visit (technician or recheck), please schedule a wellness appointment and add recommended/desired services. The Wellness Clinic Staff will add the appropriate appointment fee to the list of services when confirming the appointment.
